AKAP Care provides a range of tailored supports to help participants live safely, independently, and with dignity. Our team includes General Practitioners, nurses, and skilled support workers who work collaboratively to provide holistic care. Key services include in-home nursing, medication management, health assessments, chronic disease support, functional assessments, and coordination of care. We also offer assistance with daily living, community access, and capacity-building support, ensuring each participant receives person-centred care aligned with their goals and clinical needs.

Group and Centrebased Activities

A structured programs or activities conducted in a group settings or centres, like day programs or workshops, or art classes. This is designed to promote social interactions, skill development, and community engagement in a supervised environment.

Therapeutic Supports

Utilising specialised interventions by a qualified professional, such as physiotherapy, occupational therapy, speech therapy, counselling, psychology. This is to address specific health, mind, functional or emotional needs of the participant.

Participation in Community

Facilitate in social activities such as social, recreational. These include attending activities, joining clubs, volunteering or participating religious or cultural activities. This is aimed to increase social inclusion and connection within the community.

Daily Personal Activities

Support with routine personal care and with daily living tasks. This is for individuals needing assistance to maintain independence in their personal routines.

Assistive Products for Personal Care and Safety

Maintenance or repair of assistive technology and equipment. This includes items like mobility aids (wheelchair, walking frames), personal safety devices (alarms, consumable items).

High intensity daily personal activities

High level of support for individuals with complex needs that may include personal care tasks (showering, toileting, dressing) and clinical care needs (enteral feeding, ventilation). Require skilled staff.

Personal Mobility Equipment

Maintenance, customisation or repair of equipment to enhance mobility, e.g., wheelchair, scooters, walkers.

Assistance with Travel/Transport Arrangements

Coordinate and support with travel or transport needs that includes booking accessible taxis, arranging public transport or planning community outings. This may involve accompanying the participants if required.

Community Nursing Care

Professional nursing service provided in community settings, such as wound care, catheter, medication administration or monitoring chronic conditions.
